Planning for the future

Communicating your wishes
Advance care planning is a process of reflection and communication to let others know your future health and personal care preferences in the event that you cannot speak for yourself. This will involve discussions with your family and friends and also with your health care providers. It is especially important to talk to your substitute decision maker.

You may never need your advance care plan, but if you do your loved ones will know your wishes. Print off an advance care planning workbook.
