Referral process

Our medical staff specialize in the diagnosis and treatment of thromboembolic disorders, the management of thrombophilic conditions and issues related to the use of antithrombotic therapy. Patients referred to the VTE Clinic will be triaged and assessed by a staff physician. This is a consultant clinic in which patients are reviewed and assessed over three to four visits and then returned to their primary care-giver for ongoing care. New referrals of all types are welcome.

When a patient is referred to a specific staff physician, every effort is made to honour that request.

Please Note:

  1. Referrals can be sent to the VTE Clinic via fax 416-864-5714.
  2. Once the referral is triaged, an appointment will be faxed to the referring physician. Please contact the patient with the appointment details and have them call to confirm their appointment.
  3. Be sure to send us any pertinent test results and/or consult notes, including radiology reports (ultrasound, CTPE, V/Q scan), and results from previous hypercoagulable testing.
