Referrals
Neurovascular referrals can be sent to our team only by other health care professionals by completing and submitting our printable PDF referral form. Please note, we do not accept referrals directly from patients.
Once the form is completed, please fax it to St. Michael’s Hospital at: 416-864-5716.
Once we have received and reviewed the referral, we will contact the referring physician’s office to provide the patient’s appointment details. We do not contact the patient directly as this is the referring office’s responsibility.
Please ensure that when you call to notify your patient of the appointment details, also ask them to bring a CD of any imaging studies (i.e. CT scans, MRIs) to the appointment.
