Access to information

How to access your health information

St. Michael’s holds personal health information about patients and research participants. This information may include your health history and records of visit to the hospital. The collection, use, disclosure and management of personal health information are governed by Ontario’s Personal Health Information Protection Act (PHIPA). St. Michael’s is committed to ensuring the privacy of patient information. We take all necessary steps to comply with PHIPA and to safeguard your information.

To request access to your personal health records, please provide a completed and signed PHIPA access to personal health information form.

There is an administrative fee for copies of your record based on the current fee schedule. An administrative fee of $33.90 ($30 + HST) shall apply to all requestors. This includes pages 1 to 20. After the first 20 pages, any additional pages will cost 25 cents per page + HST. Please ensure that the appropriate processing fee accompanies the request.

Please note: There is no fee to release patient information to other health care facilities or physicians, for continuity of patient care.

Completed request forms can be sent to Health Records department via email, fax, mail or in-person. Payment can be made by cheque payable to St. Michael’s Hospital – Health Records Department or by credit card over the phone.
