Being prepared for the future is extremely important. The following resources can help you be prepared in case you cannot speak for yourself. Please contact your health care provider if you have any questions.

Communicating your wishes

Advance care planning is a process of reflection and communication to let others know your future health and personal care preferences in the event that you cannot speak for yourself. This will involve discussions with your family and friends and also with your health care providers. It is especially important to talk to your substitute decision maker.
