Community Mental Health Services : CONTACT Mental Health Outreach Service
CONTACT mental health outreach service is an assertive community treatment program for individuals with serious mental illness who require intensive community support due to a history of high inpatient and psychiatric emergency service use. Clients in the CONTACT program may also be homeless and have concurrent substance use issues.
Service is provided by a multidisciplinary team including psychiatrists, registered nurses, social workers, occupational therapists, a vocational rehabilitation specialist, a rehabilitation specialist, an addictions counselor and a peer support specialist.
The team provides outreach, treatment, rehabilitation and recovery support, mainly in clients' homes or elsewhere in the community. Service is intensive to prevent hospitalization and is therefore available seven days per week, 365 days per year. An on-call pager is available for crisis support 24 hours per day.
