Community Mental Health Services : Extended Care Clinic
The extended care clinic is a weekly clinic that provides followup for individuals with severe mental illness who require medical or therapeutic followup and are working on selected recovery goals. Individuals using the service are scheduled for clinic appointments and can participate in a number of psycho-education groups developed with input from the clientele. In addition to the clinic services, individuals may participate in a weekly self-help group organized by the clients themselves.
The clinical team consists of psychiatrists and a social worker. Self-help groups are led by clients.

- medication followup
- individual counseling and recovery plan development
- psycho-education groups
- self-help
