Programs & Services

This multidisciplinary team provides psychiatry assessments of patients referred from family physicians and community agencies across southeast Toronto. The service offers a collaborative care model whereby adequate assistance is offered to enable the referral source to manage the mental health issues. If appropriate, short term interventions or referrals to one of our specialized treatment programs will be facilitated.

The assessment clinic team is composed of psychiatrists, psychiatric residents and social workers. The team provides consultation, short term interventions and facilitation of referral to one of our treatment programs if appropriate.

Referrals

To access the service, there is one standardized referral form that can be obtained by calling the intake coordinator at 416-864-5120.
