Patient Information: Test Fees

Ontario residents covered under the Ontario Health Insurance Plan (OHIP): All laboratory services at the hospital (excluding tests which are ordered as part of a research study) are covered under your OHIP insurance. Please bring your requisitions which have been given to you by your doctor when you come to the hospital. You will also need your St. Michael's Hospital Card (if you have one) and your OHIP card.

Refugees: All laboratory services at the hospital (excluding tests which are ordered as part of a research study) are covered under your valid refugee certificate. Please bring the requisition or requisitions your doctor has given to you when you come to the hospital. You will also need your valid refugee certificate and your St. Michael's Hospital Card (if you have one).

Out of province (other non-Ontario Canadian province or territory): Please bring your provincial health care card along with your test requisitions. You will also need your St. Michael's Hospital Card (if you have one).

Patients covered by travel insurance, private insurance or another government plan: Please bring along your health care and travel insurance documentation and your test requisitions. You are responsible to pay any amounts not covered by your plan. Please note that not all services are covered by insurance plans. You will also need your St. Michael's Hospital Card (if you have one).

University Health Insurance Plan (UHIP): Please bring your UHIP documentation and your test requisitions. You will also need your St. Michael's Hospital Card (if you have one).

All other patients including non-residents of Canada: Patients who do not have OHIP or other coverage will need to pay for their laboratory tests. These charges may be paid for in the billing office (cash office) on the first floor of the Donnelly wing before having blood drawn. Please be sure to bring your St. Michael's Hospital Card (if you have one) and your test requisitions.
