Dr. Ramesh Prasad, Dr. Darren Yuen, Dr. Jeff Zaltzman are doctors specializing in kidney transplant. They review patients’ test results during the work-up process, answer questions, monitor how the kidney is working after transplant and adjust medications.

Dr. Phil McFarlane, Dr. Jordan Weinstein and Dr. Jeff Zaltzman are doctors specializing in kidney disease who work up potential kidney donors to decide if it is possible for them to safely donate a kidney.

Dr. Monica Farcas, Dr. Michael Ordon, Dr. Kenneth Pace and Dr. Robert Stewart are urologists who perform the kidney transplant surgery, are involved in the donor and recipient work-up process and who manage surgical care post-donor and transplant surgery.
Nurse practitioner (NP)/Certified diabetes educator (CDE)
Tess Montada-Atin specializes in taking care of transplant patients with diabetes. She also works up potential kidney donors. Nurse practitioners are nurses who have additional training that qualifies them to independently diagnose and treat medical problems.
Pre-transplant coordinators (RN)
Michelle Gabriel and Meriam Jayoma-Austria are nurses who coordinate the transplant evaluation process for patients who may potentially get a kidney transplant. Kathryn Salvatore is a nurse who coordinates care for patients who are on the waiting list for a deceased donor transplant.
Post-transplant coordinators (RN)
Kevin Bradley, Thelma Carino, Jennie Huckle, Imelda Lo, Sarah Mattok, Galo Meliton, and Fernanda Shamy are nurses who look after patients’ health after transplant. They work closely with the transplant nephrologists.
Living donor coordinators (RN)
Kevin Bradley and Maureen Connelly are nurses who coordinate the potential kidney donor’s work-up process. They work closely with the donor nephrologists and transplant surgeons.
Francine Kwee and Elena Nazvitch teaches patients about their transplant medicines. She reviews and monitors patients’ medicine regimens on a regular basis to make sure they are receiving the best possible therapy.
Social workers (RSW)
Suela Cela and Sharon Lee provide support by helping patients plan for and transition to a new life after transplant. They can link patients to community resources, drug coverage plans and provide information on ways to cope.
Dietitian (RD)/certified diabetes educator (CDE)
Jenny Accettura works with patients to manage a healthy diet after transplant. She also runs a weight loss program to help people lose excess weight before transplant.
Administrative assistants/clerical staff
Orlene Bailey, Sheila Buencamino, Katelande Joseph-Tavares, Susie Par, Kamisha Richards, Mona Udit and Patricia Zambri work with the rest of the team to schedule patients appointments, manage referrals and organize lab results.
Research coordinators and assistants
Michelle Nash, Lindita Rapi and Weiqiu Yuan coordinate research studies about transplant. They may approach patients about volunteering to participate in a research study.
Lucy Chen coordinates projects to continuously improve the quality of care in the transplant program. She co-chairs the Patient and Family Advisory Council where patients can provide input about their experience.
Clinical leader manager
Dana Whitham manages the day-to-day operations for the transplant program. She ensures that the whole team is well equipped to provide excellent care for patients.
