Borrowing Internally

Internal borrowing refers to the transfer of films within St. Michael's Hospital premises. Each borrower must have a barcode issued to them. Barcodes are to be affixed to the back of your hospital ID badge. This barcode must be presented every time you sign out a film. Please note that no exceptions will be made.

To obtain a barcode, you must have a valid hospital ID badge and fill out a Request For Borrowing Privileges form and return it to the film library. A barcode will be issued to you at that time. A Request For Borrowing Privileges form can be picked up in the film library during regular hours of operation.

Borrowers will be responsible for the return of films to the Film Library within the stated time period. Borrowers will be held accountable for any films not returned within the loan time or any films relinquished to a third party.

Individuals not returning films will have their borrowing privileges revoked until the films in question are returned.

Borrowing Externally

Patients requesting to take their films and reports outside of the hospital are required to phone the film library at 416-864-5662, 24 hours prior to picking them up. You will be asked to provide the following:

  • patient's name
  • doctor's name
  • doctor's address

When picking up the film and reports, an Authorization To Release Films From Film Library form will need to be filled out.
