Your appointment may be a day or evening appointment. Please check your booking time to ensure you arrive at the correct time.
After 9 p.m. all entrances to the hospital, except for the Queen St. Entrance, are locked. If you have an appointment after 9 p.m., you must use this entrance.

We do not accept bookings from patients directly.
In order to book a test we must have a completed and signed requisition (referral) from your physician.
The requisition should contain your name, your date of birth, your address, the specific test being requested, the reason for the test, and any other information that would help us provide you with better service.
Requisitions can be faxed to the number above and our reception staff will contact you for an appointment.

After hours requests are done only on an emergency basis and must be approved by the radiology resident on-call.

To book an appointment during regular hours:

A completed pink requisition with patient data, type of examination and doctor's name is required.
The bookings clerk or ultrasound receptionist can be reached at 416-864-5885.
Arterial doppler studies are performed in the Vascular Lab on the seventh floor of the Bond Wing 416-864-5890.
Those physicians requesting a special procedure or portable ultrasound should contact the radiologist scheduled in ultrasound (Ext. 5799).

Plain X-Ray examinations do not require an appointment.
Patients are seen in the order in which they arrive, however, certain examinations can only be done in specific rooms.
Waiting times vary depending on which room your examination is to be carried out in, and if there are any emergencies.
