  • Some appointments are available outside of business hours including evenings and weekends. Please check your appointment carefully to ensure you arrive at the correct time.

Please note: All medical imaging requests must contain the following information:

  1. Your full name
  2. Your date of birth
  3. Your address
  4. The specific test being requested
  5. The reason for the test
  6. Any other information that would help us provide you with better service
