Information for patients

Did you know that your social conditions have a big impact on your health?

Some of these conditions include income, employment, education, housing, gender, race, and early childhood experience. These factors are even more important to your health than your genes.

The St. Michael’s Family Health Team has created several programs to help patients with these factors, which we call “social determinants of health”.

All children age 6 months to 6 years will receive one book per year at their well child visit.

If your child did not get a book, please tell your doctor or nurse.

Trained members of our team can help people with low income or financial problems.

You can ask your doctor for a referral.

Our team includes a lawyer, who can give free legal advice, through our “Health Justice Initiative”.

Legal problems can include housing, employment, immigration, and more.

We also offer many special services to patients. Some examples are free tax clinics, workshops, group therapy and community kitchens.
