Primary Care Strategy Initiatives
The Mid-East Toronto Sub-Region is a local health planning area (see map) which supports the development of integrated local systems focused on better health outcomes for local residents. It brings together primary care providers, hospitals, community service organizations, community mental health and addiction agencies and patients/clients to develop a careful understanding of local needs that informs the development and implementation of initiatives that allow providers to deliver more integrated care for their clients/patients. In other words, it is fostering coordination and integration to improve care outcomes for the population and patient and provider experience.
